Credential Requirements

• ID or Driver's License (copy of front & back)
• Social Security Card (copy of front & back)
• Professional License (copy of front & back)
• CPR/BLS Card (copy of front & back)
• Specialty Certifications such as:
  • ACLS
  • PALS
  • NRP
  • MAB
  • Fire/Safety Card (if any)
• Current Physical Examination
• Current TB Test with Lot Number or Chest X-Ray with complete Radiology Report
• MMR Vaccine or Titer
• Varicella Vaccine or Titer
• Hepatitis B Vaccine, Titer or Waiver
• Hepatitis C Vaccine or Titer
• Drug Screen (10 panel)
• Resume or filled Employment History Form
• References (3 References from a nursing supervisor/manager dated within 6 months to a year of employment)
